Arbor of Itasca

Admissions
to The Arbor can be made from a prospective resident's home, the
hospital, or another facility, short term, or respite stays are
available and arrangements can be made with the RSD.
After
you and your loved one have had the opportunity to tour the facility,
and you have chosen to make The Arbor your home, our staff will
assist you in completing The Arbor's admission paperwork, and
also in preparing your new room for your arrival. The Arbor accepts
Medicare, Medicaid, Private Pay, and also Respite Care admissions
for persons who meet the state eligibility guidelines.
As
required by the State of Illinois for long term care services,
any individual age 60 and over seeking placement at The Arbor
of Itasca must complete the State's admission prescreening process
before admission to the facility can occur. The prescreening process
is required so that the State of Ilinois can determine the individual's
need for nursing facility services.
The
resident and/or the responsible person is responsible for obtaining
the necessary screening and all related documents for admission
and continued stay. The Arbor can assist in coordinating or conducting
the prescreening and can provide some assistance in the establishment
of Medicaid elegibility. The following can be contacted for the
